What they did
1) Keep the working records in an Obsidian vault Claude Code can read and search. Add standing instructions that failed searches do not prove something never happened. 2) Before ending a session, ask Claude to write its own dated handoff: what happened, unfinished work and what the next session must verify. 3) When replacing a handoff, retain the older note, mark it superseded and point to the newer version. 4) Start the next session from the current records, explicitly acknowledging gaps in its context. Record corrections openly when histories get mixed up so subsequent sessions inherit the correction.
How it went
The author describes using this practice across ongoing sessions and preserving a correction after Claude confused two companions' histories.
Worth knowing
The author also runs semantic archive search, but does not provide its implementation here. The dated-note workflow is described separately.
